First Visit

Online Paperwork

For your convenience, we have new patient paperwork that can be completed at your convenience on line. Once you call the office (979) 696-1996 to schedule your new patient appointment, the link to this paperwork will be sent promptly to your email inbox. The paperwork provides us with general information about yourself, your condition and your insurance status. As a result, we require this paperwork to be completed by the evening prior to your appointment, otherwise, the appointment is subject to cancellation. We also ask that you bring your insurance cards and any xrays or MRIs on disc with you.


Cancellation Policy

For your New Patient Visit, please reschedule at least 24 hours prior to your appointment. We do assess a $50 for all cancellations or reschedules within 24 hours of your appointment time.

If the appointment is for a minor under 12 years old, please download, print this form and bring it with the child on the first visit.


Good Faith Estimate

  • You have the right to receive a 'Good Faith Estimate' explaining how much medical care would cost.
  • You have the right to receive a Good Faith Esimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • If you receive a bill that is at least more than $400 more than your Good Faith Estimate, you can dispute the bill.

History and Examination

In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent. Then, the doctor will discuss your health related problems, concerns, as well as potential treatment options.


X-ray Studies

If your problem is due to an accident, has been going on for more than a few months or is severe, your specific condition may require us to either send you out for x-rays, or it more cost effective for you, have xrays done in clinic at the time of visit. This allows us to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Once finished with the examination and all appropriate studies, the doctors will discuss your health related concerns, problems and options for care. Patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, physical therapies, and/or soft tissue therapies.


Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.


Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen for follow up within 2-4 days and is very important to maintain progress. Treatment options for your condition will also be discussed during this time.
